> designed to help health care providers and people living in Ontario find relevant information on Advance Care Planning (ACP) and Health Care Consent (HCC).
Advance Care Planning (ACP) is a process which gives someone the opportunity to consider and discuss with family and friends their wishes and desires concerning future personal care options. This can be helpful in guiding their Substitute Decision Maker if they ever become incapable of providing consent for health or personal care decisions themselves. The Health Care Consent Act in Ontario requires Health Care Providers to obtain informed consent in order to give or withdraw treatment and ensures a process by which everyone in Ontario has access to a Substitute Decision Maker.
The resources feature a wide variety of formats such as decision support materials, handbooks, guidelines, handouts, presentations, publications, education and more.
