Service Description: Community home support services * volunteer-based 
Care For Caregivers - Peer support and discussion for caregivers and family and friends to share coping strategies and information on resources
Friendly Visiting - Volunteer provided on a one to one basis to visit on a regular basis
Home at Last - Hospital transition program - Transitional supports when being discharged from hospitals in Northumberland County.
Home Help and Maintenance Program - Regular, on-going assistance with routine home tasks such as light cleaning, shopping and meal preparation, and respite services * service is delivered by brokered workers - they are not employees of Community Care Northumberland; they work independently, therefore services are dependent on finding the appropriate service worker for the client including client's with dementia
Hospice Palliative Care Program - Visiting Hospice Service - Trained volunteers provide emotional, psychological, spiritual and social supports
Meals On Wheels - Nutritious hot or frozen meals are delivered during the noon hour by volunteers to the individual's home * special diets can be accommodated including diced 
Personal Distress Alarms - Medical alert system * allows older adults and people with disabilities to call for assistance using a help button * 24-hour emergency response
Specialized Transportation - Accessible & Rural Services - Escorted transportation to medical appointments, shopping and various social activities and programs
Supports for Caregivers - Assistance in meeting the needs of caregivers with their on-going responsibilities associated with caring for a loved one, a friend or a neighbour * not able to provide respite services for persons with dementia
Telephone Security Checks - Regular telephone contact provided in order to provide reassurance and, if necessary, to arrange for assistance
Wellness Programs - Social and recreational activities * leisure sports * hobby based clubs * men's group * ladies group * and fitness programs * programs vary from office to office and from year to year * the program is driven by the needs and interests of the participants and availability of facilitators and suitable venues
