Service Description:

Health centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups
Community-led health teams develop programs to improve the social supports and conditions that affect long-term health
Volunteer Program - Promotion of involvement of volunteers at all Kingston Community Health Centre sites
Prenatal and Obstetrical Care - For registered clients of the centre * people who are not a registered client of the centre, may receive obstetrical services through the sexual clinic that is also offered

Clinical Dietitian
- Nutritional information and education * one-on-one, couple, or family sessions for people with food and health-related concerns

Community Health
- Community health team works with community members on well-being initiatives * initiatives include food security, intergenerational programming, active living, community beautification, and dental care access * Community development team includes practical assistance worker, community development workers, Friendship Blooms facilitator, older adults facilitator, and volunteer facilitator

- One-on-one, couple, and family counselling to people living in north Kingston who are also patients

Self-Management Program of Southeastern Ontario - Living Well Programs
- Coordinates self-management workshops for people living with a chronic condition and/or chronic pain, their caregivers and those at-risk for developing a chronic condition.* Coordinates Choices and Changes and Brief Action Planning Workshops for health care providers * Offers other one-time workshops that support self-management skills. Builds regional capacity to continually offer the workshops by training workshop facilitators * Is creating and facilitating a regional Communities of Practice for self-management programming

- Nurse practitioners provide confidential, non judgmental, sensitive to sexual orientation, health clinic services * free pregnancy tests as well as discussion about options related to pregnancy * screening and treatment for sexually transmitted infections * Pap screening clinic at least once a year
