Advanced Foot Care - Advanced foot care services
Diabetes Education Program - One-on-one or group consultation * monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff * grocery store tours * community presentations
Salmon River Health Link - Work with individuals * complex conditions to identify needs * help navigate the system, and connect with the care * Coordinated Care Plans are developed from the client perspective * clients identify goals/priorities to be integrated with their medical and service plan of care supporting a team approach to care; promoting, collaboration and service integration * by Referral only
The Napanee Chill Zone - Youth access to community service providers, including health nurse, social worker, aboriginal teachings, counselling, community counsellors, and support workers * education/support related to healthy eating, smoking cessation, sexual health, healthy living * harm reduction education and supplies, food and clothing, community referrals, outreach, recreational opportunities, access to internet, plus programs, training, and volunteer opportunities * Napanee Youth Space has youth engagement workers on staff and access to most services through the Napanee Community Health Centre

Includes health care, counselling, health promotion, and illness prevention programs (not a walk-in clinic) * health team includes doctors, nurses, nurse practitioners, dietitian, diabetes educator, social worker, physiotherapist and Aboriginal nurse practitioner

Counselling Services -
One-on-one, couple, and family counselling * short-term and long-term counselling * group counselling programs encourage and promote skill development in the areas of self-care, wellness, and the management of powerful emotions * confidential (open to people residing in catchment area)

Living Well Education and Exercise Program - Sessions to learn ongoing self-care for people living with chronic diseases * includes information about coping, health guidelines, medication usage, working with health professionals, and informed treatment decisions (for registered clients referred by health care provider)

What is Aboriginal Health? Program
- Four week discussion series * explores how traditional First Nations approaches to health might be applied today
