Service Description: Community Support Services provides a range of services to individuals and families who are experiencing challenges in managing chronic diseases, who have a terminal illness and/or barriers to accessing healthcare. The program also supports the elderly who are frail and/or facing social and other types of isolation. The following programs are offered: 
  • Medical Transportation
  • Friendly visiting
  • Security Checks
  • Caregiver Support
  • Community Support (Advocacy & Referrals) 

Eligibility - Population(s) Served: Elderly, physically disabled, chronically ill individuals such as HIV/AIDS, Hep C, dialysis patients, blind * however priority is given to individuals and families of Aboriginal descent * application to all programs
