Service Description: An initiative by the Ministry of Health and Long Term Care, aiming to improve health care for individuals experiencing multiple, chronic, health conditions * individualized coordinated care plans are designed by care providers and their patients in an effort to increase communication and promote collaboration within all members of a patient's care team * patients feel more supported, making it easier to navigate through the health care system
 
Caregiver Café - Caregiver support group * information about community supports * voluntary information and experience sharing * caregiver education from guest speakers * a time away for caregivers

Application: Referral required from a health care provider such as a Primary Care Physician, Nurse Practitioner, Health Link Care Coordinator, or Allied Health Professional
Eligibility - Population(s) Served: Individuals who are experiencing 4 or more chronic and/or high cost medical conditions; individuals whose hospital utilization is increasing; clinical judgement of health care provider
