Service Description: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * referrals to other community services

Paternity (DNA) testing - DNA testing provides confirmation or exclusion of a genetic relationship * testing is conducted in a confidential environment and is simple, painless and non-invasive
 
Exercise and Falls Prevention Program - Group exercise and falls prevention classes * aims to improve strength and balance to prevent injury and falls
 
Home Maintenance and Repair - Referrals to homemaking as well as local services such as snow removal/shoveling, yard work, window washing, house cleaning, grocery shopping, and minor repairs
 
Phillips Lifeline Program - Medical alert systems * allows older adults and people with disabilities to call for assistance using a help button * 24-hour emergency response * the new GoSafe System offers personal security while users travel anywhere in Canada
 
Stroke Specific (Recovery) Exercise Program - Brockville - Exercise classes for stroke survivors * caregivers welcome * based on the guidelines for community based exercise programs for people with stroke Classes are held at the Brockville CPHC office at 2235 Parkedale Ave
 
Stroke Specific (Recovery) Exercise Program - Perth - Exercise classes for stroke survivors * caregivers welcome * based on the guidelines for community based exercise programs for people with stroke Classes are held at the Perth Lions Club located near the Perth Fairgrounds at the corner of Halton and Arthur Streets
 
Van Just For Seniors - Outings for older adults * van picks up at the passenger's door within Westport, Portland, Elgin and Lombardy for day trips to Smiths Falls for shopping, medical appointments, social activities and dining

Volunteers are needed for many CPHC programs and work in different areas throughout Lanark, Leeds & Grenville * contact volunteerism@cphcare.ca
