Dental Services - Dental assistance for low income families and individuals * funding is available until March 2016 * limited funding for dentures
 
Depression Education and Support - Support group * for people experiencing depression or anxiety * learn new skills, develop new friendships * facilitated by a social worker in partnership with Leeds Grenville Mental Health
 
Diabetes Wellness Program - Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff
 
Good Food Box - Food cooperative providing fresh fruit and vegetables at wholesale prices at various host site locations including:

Living Well with Chronic Pain and Chronic Conditions - Self management program on living with chronic conditions * group leaders have experience living with chronic health conditions * six week program
 
Lung Health Services - Control of respiratory needs and managing new symptoms including:
  • prevention
  • client assessment
  • spirometry testing (breathing tests)
  • education
  • and follow-up for improved respiratory care.
  • smoking cessation counselling
 
Parenting Support Groups - Education and support for parents by a dietitian, nurse, and community health worker at two locations Portland and Westport
 
Primary Health Care - Health care providers and services for registered clients * team includes doctors, nurse practitioners, chiropodist (foot care specialist), respiratory therapist, dietitian, nurses, social worker, foot care nurses and health promoters
 
Programs for Youth - Various programs offered throughout North Leeds at local schools, community halls, and libraries
 
Senior Exercise Program - Staying Fit - Exercise program lead by trained volunteers at the Portland Hall
 
Seniors WrapAround of North Leeds - One-on-one supports for coordinating services
  • personal advocacy
  • filling out forms and accessing income tax clinics
  • interventions for people facing homelessness and critical life changes, abuse, or isolation
  • referrals to other community support services as necessary
 
The Early Years - Parenting support and education for pregnant women and parents
 
What's Cooking in Westport? - Teaches new cooking skills * participants take home food * child care provided * located at the Free Methodist Church on Bedord St in Westport
