As the Palliative Pain and Symptom Management Consultant (PPSMC), you will deliver consultation, case-based education and mentorship to service providers of palliative care in a variety of care settings throughout South-Eastern Ontario. You will help build capacity amongst front-line service providers in the delivery of palliative care, as well as collaborate with community partners in educational initiatives. You will act as a key community resource in the provision of high quality, evidence-based palliative care services. 

The ideal candidate will demonstrate both passion and clinical expertise in the provision of palliative and interdisciplinary care.

Position:  Palliative Pain and Symptom Management Consultant (PPSMC)

Region:  Hastings

Description:  Responsibilities:

  • Offer consultation to service providers in person, onsite, by telephone, by videoconference or through email regarding care, e.g. assessment and management of pain and other symptoms; 
  • Link providers with specialized hospice palliative care resources; 
  • Assist service providers in the application of the Model to Guide Hospice Palliative Care, assessment tools and best practice guidelines; 
  • Provide case-based education and mentoring for service providers incorporating principles of adult learning; 
  • Collaborate with community partners in educational initiatives; 
  • Help build capacity amongst front line service providers in the delivery of palliative care; 
  •  Participate on relevant committees and task forces of the Southeastern Palliative & End-of-Life Care Network; 
  • Contribute to data collection, quality improvement programs and program evaluation; 
  • Contribute to the development of policies and procedures; 
  •  Identifies and participates in ongoing professional development and staff education; 
  • Performs other duties that may be assigned. 
Qualifications Required: 
  • Active registration with the College of Nurses of Ontario with a minimum Baccalaureate in Nursing, except in extraordinary circumstances; 
  • Masters prepared candidates preferred; 
  • CNA Certification in Hospice Palliative Care Nursing essential, or equivalent 
  • Membership with RNAO an asset; 
  • Minimum five years' experience in palliative care in a variety of care settings; 
  • Front line Palliative Care nursing experience essential; 
  • Vehicle and valid Ontario driver's license is required. 
  • Highly developed communication, organization, coordination and time management skills; 
  •  Demonstrated knowledge of nurse's role in palliative care and commitment to interdisciplinary care; 
  • Demonstrated skills in problem solving and critical thinking; 
  • Excellent interpersonal and teamwork skills; 
  • Demonstrated commitment to continuous learning and professional development; 
  • Computer proficiency; 
  • Demonstrated knowledge and expertise in mentorship and case-based education; 
  • Knowledge of community and LTC sectors

Salary:  $36.26 - $43.00 / hour

Terms of Employment:  70 hours bi-weekly
