Rapid access to assessment, diagnostic testing and treatment for patients identified as high risk for stroke * lead by Dr Nida Riaz
- consultations with internists
- a prevention nurse
- dietician
- referral to diabetic education program
- cholesterol class
- referral to other services
- diagnostic tests as needed
