Service Description:

Community rehabilitation for stroke survivors for up to 12 weeks following discharge from hospital to home or Long-Term Care
Services may include physiotherapy, occupational therapy, speech language therapy, and/or social work

Patients discharged from hospital who have survived a stroke and have a valid OHIP
