Service Description:
|
A program that provides housekeeping or housekeeping assistance to people who have recently been in hospital or have a chronic health problem
- arranges for a local worker or company to provide the services
- conducts interviews and security checks with local professionals
- maintains a list of workers who can meet most housekeeping and maintenance needs
- connects customers with a worker and organizes the schedule directly
- follows up with the client to ensure that he or she is satisfied with the services provided

Eligibility / Target Population:
|
Individuals 60 years of age or older who have addictions related to the aging process, previous illness or injury, permanent or periodic impairment, or physical
disability Individuals between the ages of 18 and 59 may be eligible if:
they have a physical disability or a debilitating chronic illness OR
they are clients of Hospice Quinte ET
their family/friends/caregivers are not available or are unable to provide services AND
there is a demonstrated need for Community Care for South Hastings services
