Service Description: Service navigation, referral, support, education and resources for persons living with all forms of memory loss and dementia including Alzheimer's disease and their partners in care * helps people remain independent in their own homes for as long as possible

No referral or diagnosis required to receive service and support.

Fees: Free with the exception of some specialty programs/events
Application: call/walk in/self-referral or referral from health care professionals

Language Notes: Resource materials are available in a variety of languages
