Medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurses, nurse practitioners, and health educator
Services:- diabetes and cholesterol classes
- chronic obstructive pulmonary disease (COPD) program
- smoking cessation program
- on-site lab services for registered patients (appointment required)
- postpartum depression
- breastfeeding teaching and support
- family planning, pregnancy tests, birth control, sexually transmitted diseases
