Service Description: Family and community health care team* coordinates patient care * team includes family doctors, nurses, nurse practitioners, dietitians, social workers, occupational therapist, diabetes educator, and foot care nurse * after hours care available

Diabetes Support - Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications and lifestyle choices * registered nurse and dietitian on staff
 
Health Education Programs - Diet and nutrition, fatigue, and falls-prevention education programs
 
Telemedicine - Virtual Care - Appointments with a specialist or health care provider using a two-way secure videoconference technology * allows patients to access a specialist anywhere in Ontario * central site for coordinating the appointments for residents of Kingston, Frontenac, Lennox and Addington counties

Fees: Medical services may be covered by OHIP * some fees for other programs
Application: Call or visit * appointments required for some programs * no referral required
Eligibility / Target Population: Clients who already have a family doctor cannot be taken on as clients at this time
