Service Description: Health centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups

Doctors, nurse practitioners, nurses and other healthcare professionals work together to provide primary health care services including physical assessments and treatment, nutrition support and social work counselling to registered clients

High Risk Foot and Wound Care - Wound and foot care for people with high risk chronic illness and advanced foot problems * provided at the Centre

Nutritional Counselling - Information and education from a clinical dietitian

Oral Health - Emergency dental care and preventative (hygiene) services for qualifying people under the age of 65 with no access to any other forms of dental benefits such as private insurance or other government programs * some restrictions apply

Telemedicine - Coordinates Ontario Telemedicine Services (OTN) throughout Hastings and Prince Edward Counties

Thrive - Support for mothers with addictions - Supports for pregnant or parenting women (children ages 0-6) struggling with substance use
