Service Description: Interprofessional team providing high intensity services in the community

Individualized treatment * longer term services * linkages * monitoring and evaluation to individuals with serious mental illness * also known as a hospital without walls with high level services

Eligibility / Target Population: Individuals with serious mental illnesses that are complex and have very significant functional impairments and who because of limitations of traditional mental health services, may have gone without appropriate services * as assessed by a Case Manager according to set criteria
